See the wavy tail and crooked hip? (Where the body ends and tail starts). Does he have any lumps in his back or an underbite or anything in his jaw? It's hard to tell in these pics. I would give him a mixture of calcium, a tad CGD, and water and let him eat it off your finger or a popsicle stick or something. Start today and Do that once a month. My geckos love cal, so hopefully he will too. Every seventh feeding or so, add extra cal. Give Home insects with cal. Use cal+d3. You could also provide an uvb light, if you want. What does he weigh now?

Wow he really gained some weight. I can definitely see how he got MBD! You can also put a bowl of dry cal powder into his cage and he'll eat it as needed.

Not being fed enough. Wrong diet. Not dusting crickets. Crickets can cause parasites, not MBD, unless that's all he was fed. My first geckos were only fed meal worms and crickets for two years, luckily they were dusted. But they are healthy now!

It could also be from mother not having enough calcium when laying eggs.

It's not a severe case of MBD. You might be able to straighten the tail out a little, but it won't go completely straight. Just stop it from getting worse. He is beautiful by the way. Seems like you're doing the very best you can and he should live a long happy healthy life.
