That's very odd. Is there any bleeding or is it just swelling? Is she proven or is she still a virgin? I have little knowledge of crested disease and infection so hopefully Dr. Alan will help you out :) .

And how do you know she's a virgin? We want to know.

Sounds, er.....smells (I guess) and looks from the pics like an abscess. I'd be inclined toward hot packs and baytril 1/100th ml q48h (every two days).

whoa there u lost me. do what with the hot pack's and how much of what ?

whoa there u lost me. do what with the hot pack's and how much of what ?Looks like an abscess. Needs drained, hence the hot packs (wash cloth; hot water), broad spectrum antibiotic can't hurt.......

it was a blockage ! i was looking closely at the area and noticet that from where the first infection was the "area" had healed wrong. basicaly it had healed over to where there was barely any opening. and i could see some blockage undrneeth. so i performed an emergency ummmm widenig of the uhhhh thingy. and was able to work out a LARGE fecal blockage. there was a little bleeding and i could feel that she was in alot of pain but it had to be done to save her life.

what aftercare should i do to ensure a healthy recovery and to avoid the same thing from happening again ? (antibiotic's are a given)

i must admit the whole operation was a bit unnerving. and deffinetly disgusting !

Gotta admit, I'm puzzled. Sounds like stool was possibly accumulating in the anal gland area rather than being passed normally, or that the previous abscess somehow healed with a shortening of the cloacal opening (vent) so that the gecko couldn't properly "take a dump", resulting in fecal material accumulating under the skin. Either way, what you did will probably resolve the problem short term, but my concern is that it will happen again since the animal's natural healing processes will tend to close the surgical opening that you made (I think you did sort of a primitive colostomy  ). Best advice I can give you is to keep the area clean and moist and apply a topical antibiotic (bacitracin or neomycin) for the time being, and get her to a veterinarian when you can. Good luck.

Addendum: I recently treated a male crestie who presented looking much like your pictures, except that his swelling was clearly involving the hemipenes; the swollen area was south of the vent. He had an abscess and when the hemipenal area was lanced, we got out a couple of grams of pus: not feces. Implanted a latex tube called a penrose drain for 10 days. We are now several weeks down the road and thusfar, knock wood, he seems fine and normal. That case was different because the material accumulating was not stool and because the vent was never compromised.

That is a very un-fun case of constipation! Max, I'm sure it was pretty unnerving, but I bet she's a lot more comfortable now without her vent about to explode. The only thing that I DON'T like about working with animals is that you can't tell them that it will hurt now, but make them feel better afterward.

Dr. Alan, that male with the infected hemipenes, did he get systemic (oral) antibiotics, or just topical antibiotics on the area? Also, did the owner have to flush the penrose with saline, or was it just there for natural drainage? It must have been one very tiny piece of penrose drain tube, too!

None of my pets have ever had an abcess (thank goodness) but I was curious.

Also, did the owner have to flush the penrose with saline, or was it just there for natural drainage? It must have been one very tiny piece of penrose drain tube, too!Just cut a standard 1/4" penrose cut in half lengthwise and sutured in place. I rarely if ever flush penrose drains.

 Oooooh, I wouldn't like to do injections. I would if I had to, but :| eek. Does the owner get shown how or does the gecko have to come in every other day?

The penrose drains I barely remember from when I used to work at the vet's office as a teenager. I remember a couple being flushed but it must have been for something really specific. (I remember them having to use a penrose on someone who let their dog's choke chain grow into its neck, though. )

I hope she gets better, Max!

well i just went to put on the antibac ointment and she look's much better. the swelling has gone way down, beyond what came out last night. but she is deffinately afraid of me. she started backing away from me the second i opened the tub, something she never did before. i feel bad that i hurt her, nad that i have no way to let her know that im just trying to help her to feel better.

Actually that particular gecko was shipped to me for treatment and I still have him. He'll probably go home in a week or two. With a local client, I probably would have had them give the baytril orally. Honestly an injection is less work and much less stressful for the gecko than cranking the mouth open for oral medication although baytril is equally effective either way.

she started backing away from me the second i opened the tub, something she never did before. Probably because of what we in the profession jokingly refer to as the "Bulgarian anesthesia" that was employed for her er.... procedure.

i feel bad that i hurt her, nad that i have no way to let her know that im just trying to help her to feel better. You did what you had to do, and relieved the acute problem and bought her some time. Let me know what your veterinarian says after he sees her this week. Glad she's looking better.
